Insurance
Office Policies
For an appointment or consultation, contact
the office of Dr. Stacey Johnson, Dr. Hassan Albataineh, Dr.
Hicham S. Merheb, Dianna Owen and Marion Conway Arnold. (A referral
from your family physician is not
required to schedule an appointment. However, please note that
some insurance carriers do require a referral. In such instances,
please obtain a referral form from your primary care physician.)
The office staff is available to discuss payment and medical
insurance.
When you visit the doctors’ office for the
first time, please allow 60-90 minutes to discuss your medical
background with the physician and undergo a physical examination.
Insurance
The clinic participates in Medicare, accepting Medicare
assignments, Arkansas Medicaid, and most
insurance plans. Patients are requested to contact their insurance
companies prior to their appointment to confirm that insurance
requirements are met. (Some insurance carriers may require a referral.)
In such instances, you will be informed you need to obtain a referral
form. The office does request file claims with the patient’s
insurance carrier. We do not accept payments by credit cards. Payment
is expected at the time of service, most often payment is the insurance
co-payment or deductible.

site! Or, if you have concerns about providing information via
the Internet, fill them out, and print them prior to an appointment.
Then bring them with you to your first visit to save time when
you arrive.
